Pharma commercial leadership team reviewing an omnichannel sales plan in a boardroom

The 6:1 ROI Nobody Wanted to Fund

The best tactic isn't the highest ROI. It's the one that moves the brand's number.

Key takeaways

  1. Contribution beats efficiency. A 6:1 return on $100K is $600K, only 0.4% of a $150M target. Pick the tactics that move the brand's number at an acceptable cost, not the highest ROI.

  2. Five things well beat twenty halfway. Target the HCPs you can win, coordinate field and non-personal promotion, test small and scale what moves new patient starts. Engagement, not reach, is the leading indicator.

  3. Get the outside read before the money moves. 80% of pharma executives are content with their engagement strategies; only 35% of HCPs say their needs are met. Pressure-test the approved plan before the first dollar is spent.

Early in my career I ran digital marketing for a large pharma company. I had tactics with returns most teams would envy, and I could not understand why the marketing VPs wouldn’t put more money behind them. So I asked one.

His answer changed how I think about commercial work. The ROI was never the problem. Scale was. A 6:1 return on a $100,000 eMail campaign is $600,000 in revenue. Against a $150 million target, that doesn’t register. Reps and TV earn lower returns, but they carry the number.

The job isn’t to find the highest-ROI tactic. It’s to find the tactics that move the most revenue at an acceptable cost. That is the difference between activity and impact. If your 2027 omnichannel plan is built around the first idea, this piece is for you.

The goal is rarely the problem

Most brand teams can describe the goal. Fewer can put it to work. The industry data shows where it breaks.

About half of pharma launches in the last two years underperformed analyst expectations¹.

45% of HCPs were accessible to pharma reps in 2024, down from 60% eighteen months earlier. Half of those limit engagement to three companies or fewer².

65% of HCP engagements are not coordinated across sales and marketing, which slows treatment adoption³.

Nearly 80% of approved content is rarely or never used, and field teams share content in just 48% of HCP meetings⁴,⁵.

Access is scarce. The channels aren’t talking to each other. Most of what gets produced never reaches a prescriber. Adding tactics doesn’t fix that. Better choices do.

65% of HCP engagements are not coordinated across sales and marketing, a gap in omnichannel sales execution

A mandate, not a meter

This is where commercial sales leadership consulting usually goes wrong. Most firms sell time: hours, headcount, a percentage of an executive’s week. Kairos Meridian takes on a mandate.

A mandate is written. It names the audience, the window and the one metric that matters. Here is an example:

Situation: A competitor is gaining share, and the brand just won a market access advantage it hasn’t converted. Audience: “Dabblers,” HCPs who write the brand occasionally but write the competitor most. Window: Six (6) months. Mandate: Grow new patient starts in that audience by 20%.

The role flexes to serve the mandate, from strategist to operator and every point in between. One week that means building the segmentation. The next it means working with district managers on coaching content. The measure never changes.

How the plan gets built

Omnichannel sales execution comes down to four decisions: who you talk to, what you say, when you say it and where you say it. Targeting, content, timing and channels. Each one is made under two constraints, time and budget.

Start with the data, not the tactic list. For example size the HCP dabbler audience. Map patient volume, market access, market split and where reps already have access and a relationship. Go after the competitor’s dominant writers where the product has a clear advantage, not every possible switch. That kind of precision is rare: only 10% to 15% of launches attempted to personalize at the individual customer level¹. If time allows, find the quiet influencers whose prescribing creates a halo effect on peers in their area.

Buy reach at the lowest cost that still earns engagement. Look at eMail opt-ins and media outlets. Balance reach, frequency and engagement against the budget. Ask how often script data refreshes, because that sets how fast you can learn.

Do five things well, not twenty things halfway. Review what past campaigns actually taught. Cut the tactics that are in the plan only because they were in last year’s plan. Run several small tests, then scale what moves new patient starts. Treat engagement as the leading indicator, not reach and frequency. An impression is not a conversation.

The goal is simple to say and hard to do: the richest engagement with the most influential HCPs, for the fewest dollars, at a scale that moves the brand’s number.

Field sales effectiveness is where it lands

None of this works if the field sees non-personal promotion as a competing plan. It has to work for them. The rep still matters most.

HCPs give pharma an average Net Promoter Score of −10. Companies with more effective rep engagement score 22 points higher⁶.

A rep call is 30% more likely to result in a prescription when digital exposure follows within 10 days⁷. HCPs who visit a brand website after a rep visit are 60% more likely to prescribe³.

So what changes? Reps prioritize specific HCP visits, see the right prescribers more often and drive eMail opt-ins so the conversation continues between calls. District managers coach on content for specific switches, not call counts. Incentive pay for that plan-of-action cycle ties effort to the mandate.

Fewer, better conversations. Managers coaching to the metric that matters. Pay that rewards both.

HCPs give pharma a −10 Net Promoter Score; more effective rep engagement lifts it 22 points (ZS 2026)

Paid for the thinking

Kairos Meridian is paid on the value of the campaign thinking, not the number of slides, the complexity of the plan or the number of tactics. That changes behavior and ultimately outcomes. There is no reason to add complexity that adds cost, and every reason to say “don’t” when a tactic won’t earn its place.

AI and modern analytics make the decisions faster, the responses quicker and the team more agile. They are how the work gets done. They don’t do the thinking, and they don’t sign off on the answer. A senior partner does.

Before the plan goes live

Your 2027 omnichannel plan is approved or close to it. The cheapest time to change it is before the first dollar is spent.

80% of pharma executives are content with their customer engagement strategies. Only 35% of HCPs say pharma’s customer-facing resources meet their needs⁸.

That gap is hard to see from the inside. An outside read helps most here. It should come from someone with nothing to sell into the plan and no stake in how it was built. That read separates the five tactics that will move new patient starts from the fifteen that will only fill activity reports.

If you’re facing a competitive threat, an access win you haven’t converted, or field and non-personal promotion running as two separate plans, let’s talk. The first conversation is 25 minutes. It’s a diagnostic, not a pitch. We listen for the real problem, then come back with a structured read of your plan.

What does Kairos Meridian do?

We do the senior commercial work pharma brands need while the decisions are still open: building the strategy, sharpening the program and running it with the analytics to prove it. The work is our product.

What are your areas of expertise?

Commercial strategy in pharma & life sciences, with the deepest work in omnichannel, direct-to-patient, innovation, data and analytics.

What kind of businesses do you work with?

Small and mid-cap pharma brands in their first five years, the vendors selling into them, and the private equity firms holding related assets.

What don’t you do?

We don’t sell technology or media, and we don’t do the technical build: no media buying, campaign production or platform implementation, and no junior consultants doing the thinking. We take strategy through execution and bring the analytics to measure it, which keeps our work independent.

Are you a fractional executive service?

No. Fractional arrangements sell a percentage of someone’s time. We take interim leadership roles with a specific mandate, a defined goal, and an end date.

How do you price your projects?

Fixed fee, scoped to the question, the weight of the answer and operated with efficiency. Our pricing is not based on hours, headcount, and agency overhead.
